Too much super thrive or over fertilizing imo. Are you getting active root growth? If not you might want to up the humidity. I had a similar situation last year, when day/night temperature varied too...

Rescued this orchid clump from roadside, It was on a dead tree which was being sawed down. Part of it was squashed under the fallen tree and had to be discarded. One of the shoots has 3 blooming...
