Old pseudobulbs drop leaves regularly. It's not a big deal. It is a big deal if a whole bunch of leaves start dropping.
I would also try growing in a smaller pot if possible. Try growing in a 2" tall pot. If you've got a clear plastic pot, (whether it is purchased or made out of a clear plastic bottle, it doesn't matter), that's great too as you can see what's happening with the media. Mine is easily 2 - 3 times larger than yours is, and it is still in a 2" pot.
I would also not pack the sphagnum moss too tightly.
It is also not being grown bright enough. Yours is still a very dark green. Max tenuifolia is a light green color when grown bright enough. I've grown Max tenuifolia under full sun before, and it did just fine. If you grow Cymbidiums, Cattleyas, or Vandas successfully, that's how bright they grow.
