Some roots have black ends, Some are firm and green!
What does this mean?

Oh the yellow leaf that dropped off is the one nearest to the bottom, but it's also the smallest leaf.

Pictures of the roots would help. But generally if the roots are firm and either green(wet) or silvery white (dry) they are healthy. Soft mushy or hollow roots are dead and should be removed. You can cut up to a healthy portion if one is partially dead. I like clear pots for Phals because you can see the roots and when they are green, it is still wet in the pot and not time to water. When they look dryer (more silver white) they are dry and likely time to water them.

Green roots sound fine, if the dark tips are mush they are rotten but if firm then leave them.

Small leaves at bottom are generally ones that great when the plant was younger and smaller. They won't get bigger once newer leaves have grown (sometimes two may grow together but usually its just one at a time). These are still old leaves which it could be natural to drop.

The exception is if it was a keiki, a new baby plant growing from the base of the main one. Was the leaf growing from the same center point (crown) as the bigger ones or with a separate crown?

Some pictures might help if you're not sure.

I've edited you post to turn it into a link. Not sure why it wouldn't work for you, unless you had the quote marks arround it.

This sort of link can't be used ad an image as its a link to a web page not an image on the page, but the URL method should work.

I can't see yellow in the pics. The roots look OK though not brilliant. They might be very tightly packed in that moss which can be a problem. It might be worth pulling some out to loosen it. Are the roots firm?

yup all roots are firm Some have black tips though, Recently the leaves are sagging and the yellowing is way more prominent. I think it's because of the crack in the leaf? When i got it the crack was already there

Here's an update with pictures!
It's not doing very well,
I pulled out the moss as suggested, it really was packed very tight! I had trouble with it. The Moss kind of hard and crackly.
