We'll have to keep on the lookout for a reason and solution to this problem. If the outside of the plant has no fungus etc. then what??? If the roots are healthy, it likely isn't a potting issue. A very puzzling dilemma!

Sounds like stem rot. My sister lost her 3 Phals to it. At her place it's quite dark and cool, and no plants thrive there. The root system was great, but one by one the leaves yellowed and fell off until there was nothing left. She keeps her remaining Phal at the office, where it's doing great.

I just found this info. could this be happening? Fusarium wilt is not uncommon among other plants in the garden:

Many varieties of orchids including phalaenopsis are susceptible to fusarium wilt, a soil-borne fungal disease that infects the plants through the roots and spreads upward. Symptoms of the disease include yellowing and wilting leaves, stunted plant growth, premature leaf drop, purple discoloration in the rhizomes and rotted roots. Treat infected orchids with a registered fungicide to prevent the disease from spreading. Adopt good cultural practices to reduce plant stress that increases susceptibility to the fungal disease
