from my search it seems this is a Spatulata type Den - I don't grow any, so can't help with care - hopefully someone else will be able to - good luck!

Sonya is right...spatulata or antelope(curly) type of Dendrobiums....you can google the Baker's culture.

NYCorchidman brought this exact plant back to life after having some infestation of some kind; you can message and ask him yourself on how he grows it
