Urgently need help on my dying orchid

MY orchid seems to be dying.
Bought 2 weeks go.

now ended up like this.

from healthy leaves till all the leaves turn yellow and drop off.

Would appreciate if anyhow can guild me if I can still revive the orchid.

If possible what are the things i need to do to revive it.
