Default Lost the tag....and it's starting to look sad...
So I have a beautiful new orchid....Lost the tag....and it's starting to look sad...-imageuploadedbytapatalk1413593761-973138-jpg

A) I have no idea what it is....thoughts?

B) it seems to have a problem....brown, mushy pseudo bulb. Yellowing leaf. Also, missed a picture of it but a whole pseudo bulb plus baby leaves was beginning to yellow....when I went to look at it it just fell right off in my hand

Help?!?
