If you can wipe some of the black spots off with a tissue- is it possibly mold of sorts? When you received the plant, did you give it a thorough "once-over" to try and find anything out of the ordinary? If it is mold, depending upon your potting medium, if the phal is constantly wet- then the medium is wet and it can present prime mold-growing conditions. Sometimes with mold it can be "choking" your phal because they breathe through their leaves and their air roots...
I am not the most experienced person, but I would check out
Orchid Diseases to see if anything there matches up with your problems.
