A better photo would be from the side or head on i guess you would say not from above, so i can see the full picture and look for the new tissue growth at the crown to see if you are over or under-fertilizing. But looking at that one root of the few, it is very possible that you have a mild thrips problem.

Ok, I would suggest you spray for thrips, and there seems to be almost no new leaf growth and if so very slow by the looks of things. Suggests that you have an insufficient feeding regimen. A tbsp of epsom salt to a gallon of water once a month and fertilize more regularly. Every other day to start. Do several passes with the water or fertilizer to ensure full saturation of the roots. (No visible white/grey parts on the roots)
Your light seems to be great based on colour, so I would just suggest that from leaf tip to alternate leaf tip the plant is hung east to west so leaf tips on one side face east and the other side faces west. That is ensuring maximum light exposure for the plant.
Adding some calcium can't hurt either once a month. :)

Keep us updated...I'm sure it will come around. Remember you ideally want pencil thick roots and that usually comes with proper watering/feeding regimen.

Well well well.......I finally managed to get this thing to flower, after 2 years worth of continual bud-blast.

Well okay, only 1 in bloom thus far with a second possibly 3rd flower on the way.

I went back and counted all the failed spikes, I got to 18 so yes, i'm pretty happy at the moment.

Congratulations! Fantastic looking white BTW. I hope it settles down for you now and keeps blooming and not blasting.

Perhaps there are thrips? One of my friends had similar problem with vandas and now sprays developing spikes with Bayer 3 in 1. Solved her problem.
