Default neofinetia leaves keep falling

Hi folks, this shutennou was originally potted with moss in traditional style. I got this one a few months ago. Recently the leaves have been falling one by one every a few days. I took it out, removed a few rot roots, cleaned it with hydrogen peroxide and re-potted with some of the original moss. But two more leaves have fallen since then. As you can see one of the growth has essentially lost all its leaves, but the crown is actually dry and not soggy as the other crown rot I have seen on phals. So what is the cause of this and should I start to apply fungicide ?
