For what it's worth...I can't tell you how many phals in moss I've killed in the last several years. Tried everything, you name it, I tried it. Repotted loosely in new moss, watered less frequently, etc, etc, etc. Nothing worked. I was plaqued with bacteria, fungi, root rot, crown rot you name it! we had it.
I finally (duhh) switched to bark, soaked the roots in Physan prior to repotting, as James mentioned. It's been 2 yrs. and haven't had any problems since. One phal had spider mites a few months back but responded well to Bayer 3in1.
Phal roots like a lot of air circulation. Some people mount theirs on cork alone. I also introduced a Oscillating fan which helps.
