My phal is blooming but the leaves are yellowing - help please!

UPDATE: The roots were indeed rotted, pretty badly. I sliced off the rotted roots with a sterile Exacto knife and dusted the exposed ends with cinnamon powder. It's been repotted in bark, though I'm not sure there was enough root base left to salvage the plant. Thanks so much for all the helpful tips!
