Also dull bits, impatience, and choosing really, really cheap glass with fun shapes like my beer glasses and wine glasses
I think the dull bits part is the most critical, I don't know how often ya'll replace yours, but it seems like they are honestly only good for a couple vases, and so I end up needing a bit of force to keep it going, and tend to break the glass right when it "punches" through. For a while I was breaking glasses more often and realized that I didn't have my drill bit in far enough and it was oscillating enough to cause problems.
The wood shim mentioned above is also something I haven't tried, but I am now going to etch my holes with the vibra-marker I used for etching the glass numbers, and I suspect that will get me started a lot easier.
I haven't tried FULLY submerged though.
