phal dying stem question

Hello,
I've had this stem deterioration happen on 2 previous phals, and now this one. Does the stem/s just decide that it's time to die back - or, and more likely, what am I doing wrong?

I've been using these "self watering pots" lately with pretty good success - using orchiata pinus radiata bark as a medium, figuring that I could move to clay pellets if the pots worked out.

Maybe the photo can't depict the fact that just about every root has no substance - when I squeeze them there is nothing there to feel but the actual root itself....
