Phalaenopsis with yellow leaves and black spots - is it dying?

Help! My 3.5 year old phalaenopsis orchid is suddenly not doing well. Two of the leaves turned yellow about a week and I carefully removed them with sterilized shears. I thought they were just old as it has never lost any leaves, but now another younger leaf is turning yellow and seems to be dying too. :(
The other leaves also have some black spots on the tips. The white petals are now showing strange little black and red spots! The roots seem healthy and the petals and stems are nice and stiff. I am worried it's dying!! Any advice?!? My next plan is to repot it with fresh bark. |

this morning I also noticed some browning on the roots. should I cut off any roots that seem infected? and should i find a fungicide to use? (I'm very new to orchids! I lucked out and found the perfect window and watering schedule for this one, which allowed me to keep it in great shape for the last few years, but i don't know much about how to care for it when something goes wrong..) |
Those roots are still OK... but agreed, they don't look completely wonderful. When was the last time you repotted? Most people believe that kind of bark lasts about 2 years, maximum.
