Lepanthes telipogoniflora issue.

Does anybody know of anything I could be doing that causes this issue?
My lepanthes will push out two buds, which get fused together. Which causes both buds to drop, and sometimes aborts the spike.
Here's a picture with a new and mature spike that are doing it.
