Stelis nanegalensis (syn. vulcanica).
I had a pleasant surprise the other day - one of my Stelis flowered, with the spike hidden behind another leaf; as I was inspecting the plant, the spike popped out and surprised me with flowers!
This was purchased as a Stelis vulcanica, but it appears that name is invalid and it's actually Stelis nanegalensis.

Ron - I'd say this is just under 1cm (~9mm) at its widest. Best I could do with a ruler.
