Looks like Tsubotaara (Tst.), which is, Aganisia X Pabstia X Zygopetalum. Look at Tst. Melinda Marie, there are few different varieties of it. The problem with the leaf is probably related to not enough water or humidity. We keep ours in wet moss (not soggy) all the time and that seems to keep the leaf issues away. We grow it like our oncidiums but a little wetter all year. It can take warmer conditions than true zygopetalums, the summer temps can reach 100F in the shade here and has not effected it.
