Welcome to the Orchid Board!
A lot of what you see is damage from being bumped and scraped on the display stand and being handled by customers. I don't see anything to worry about.
Small Phals like this can do well in these small containers of moss until the moss breaks down or the plant outgrows the container. Then it will be time to move to a larger container. I would repot a plant at any time if there was a problem with old, decomposing medium, or the plant might have root damage. For a healthy plant like yours I would wait for warmer weather and longer days.
Be sure the moss gets nearly dry between waterings. Wet roots invites rot, which can kill the plant. If you let it get dry between waterings, but not for more than a day or so, the plant will do fine.
Large bark works well for potting Phals, but beginners tend to underwater small plants in large bark and the plant declines.
When you do water, don't completely soak the moss. Run water over the top. The water will soak down and moisten the rest of the moss. If ambient temperatures are high, and humidity is somewhat low, you might need to water every 2-3 days. If it is around 72 F / 22C and typical indoor home humidity, you won't need to water more than once a week or so.
Heft the plant in your hand when it is dry and notice how heavy it is. Water it and notice how heavy it is. That will help you judge.
