Blisters and pits on Phal leaves

Hi everyone! I seem to have a new problem with three of my phals. There are some spots on the leaves that I cannot figure out. The first photo shows (blurrily, I know, sorry!) a white spot that is raised on the leaf there. I am also concerned with this plant because the flowers are wilting suddenly, and not the oldest ones first. The two buds that bloomed after I got it never even fully opened like the rest. The second photo shows my oldest phal and it has dry white pits on a couple of the leaves. I have found and treated scale insects on this plant, but I didn't know they left pits like this, so I'm not sure that's the problem. The third photo shows another phal (I think; hasn't bloomed for me yet) that has raised blister like marks on a couple of the leaves. I would appreciate any help or suggestions identifying the problem(s) with these guys. Thanks!
