Ditto on all the comments on soil. Certain death for any kind of orchid!

Is that pot glass. It looks to me like there might not be enough aeration in the bottom, leading to it staying water logged at the bottom when it dries at the top.

I like clear plastic pots, which if necessary can have more slits/holes made to increase aeration. I've not tried glass pots.
