I don't see any mite damage , is the Phal. leaf a lower one if so it is normal to lose a bottom leaf this time of the year . Dends will lose leaves off and on I have some with naked canes, as long as they are growing new canes all is ok . Gin

Feel free to increase this to 4 tubes (T5) approx 12-20" away.) Phals can take lots more light than many think (Dennis has been preaching this regularly). I use pretty bright compact florescents for mine. They are in south-facing window with three compact florescents as suplements.
