Hey, Liz...
I just remembered. You like to put Hydroton on the bottom portion of the pot.
I think I might know why you got that problem with mold on the CHC even after you boiled the media.
Do you have Hydroton in there?
And if so...
Have you done anything about the Hydroton?

It got boiled and treated with physan as well. There was never any mold growing on media- just on the roots.

Oh...
I've had something like this before. It's very hard to cure.
It happened to the Cleisocentron merrillianum.
Well, I think it's a lot more serious than just sterilizing the media then.
I think you gotta go back and look at your cultivation techniques, and examine what needs to be tweaked.
I'm thinking it's the frequency of the watering and the air circulation. It's really hard to tell.

This is the only plant this has happened to. And only a day or so after repotting. I have not watered since it was repotted, save for soaking in physan, listerine, or H2O2.
It was repotted out of the sphag it came in- which was very decomposed, with lots of algae. I'm surprised it had the excellent roots it did.
How do YOU sanitize plants when you repot?

I usually don't have to.
When I do, I try a number of different methods that you've already done.
There's also the option of removing the infected portion of the plant.
But, sometimes if it's unsalvageable, it's unsalvageable.
Unfortunately, there are times, where it's just a numbers game. Sucks to hear, but sometimes that's just the nature of it all.

Physan or bleach for cleaning excessively dirty pots helps out.

I'm actually a little wary of using Physan on roots. I did that once, and it didn't work out in my favor.
