Sunburn.

Roots look fine.

The rest of the leaves look okay.

There seems to not be any signs of rotting or fungal infections.

But the leaves are a very light yellow green, which is very consistent with over exposure to light. And the dark markings are consistent with sunburn damage.

Solution:

Put it in a shadier area.

If you have a sheer curtain the phal would probably appreciate the protection while still getting that good light.
If not, west or especially east facing windows work really well.
I have success in southern windows, which is really strong sun, but my windows are filtered and double paned.

Also, make sure the leaves are not pressed against the window, it can get burned that way as well.
