I remember seeing this on various paphs at different times. I don't know that I ever understood exactly what the cause was, but the treatment was getting the plant growing strongly. This involved making sure the plants were potted appropriately (not too big, not too small - just right!) and frequently (once a year is about right) and that they never dried out completely or stayed too wet... All that said, some plants were always more susceptible than others to the pitting.
