I’m guessing it’s the black gold product that’s to blame for the state of the roots. Far too much fine material and it suffocated the roots from lack of air. Anything that can go 2-3 weeks between watering is way too water retentive for phals and/or the growing conditions. Phals need lots of air around their roots. If they have that, it’s essentially impossible to “over water” them.
Look up sphag & bag. It’s a low tech way to provide the plant with a high humidity environment while it attempts to recover. The plant will look worse before it looks better. It will scavenge nutrients from the damaged leaves to fuel new root growth. You can also pot it up in the bark you have and put a clear plastic bag over the top to help trap humidity. You will need to water much more often than you currently are, but still wait for it to dry out a bit between waterings.
