What have I done wrong to cause this problem?

Based on the photo, the mix looked rather broken down. When was the last time you potted the plant? Since Cyms like to stay on the damp side, bark definitely breaks down - faster than, say, for a Cattleya that stays drier. So, I find that, at least in a perfect world, Cyms really should be repotted every 3 years (sometimes less, depending on the quality of the bark). Do I meet that goal every time? Alas, no.. but I do try. If I don't the plant will tell me... as I think the plant was telling you. Choose your pot size for 2-3 years' growth, no more. You got it before serious damage was done... there is good new root growth, Cyms tend to be root machines.

I don't think you've done anything wrong. It might have been ready for repotting a while ago, and you could have fertilized more.

The orchid was in the pot for 3 years. Most of my Cyms are due for a repot as soon as the finish flowering. That is probably poor planning.
