Default Beallara Tropic Tom 'Pale Face' in need of your help!

Hello everyone! I am in need of help with this plant. I got this plant from a local nursery less than a year ago in spike and it produced beautiful flowers. Unfortunately the pseudobulbs were not in the best condition when I purchased it and decided once it was done flowering I would replant. I found that there were chunks of sponge-like material in the center of the plant and some of the smaller pseudobulbs were deep inside the media. I replanted in a mixture of perlite, charcoal, fir bark and a little sphagnum moss. The plant started to put out a new growth, but then it turned brown and died. Also, the 2 final leaves are now starting to turn brown. It lives in an east facing window, which currently is pretty chilly due to the Midwest winter. I need help bringing this plant back to life. There are still some healthy roots and I have hope!
