I believe your plant is dying. This might have been caused even before you got it. I'm not sure about this, but I think it looks like the same type burn that I saw from feeding ice to orchids. The stores might have been caring for it this way previously. It takes a little while to start showing the damage from it. The keiki might be it trying to survive, so it's throwing out a new plant. Can you check the roots? If they are mostly black or mushy (it's not good), then those need to be cut off.
Perhaps someone else will chime in here with opinions.

i recently checked the roots, when i took the pictures last night. and that little yellow growth is a keik? thank you so much for your help. ......and to anyone else, please please feel free to post

i was also wondering what i should do now. and i thought it was a good idea to inform you that about a month ago it grew four new small leaves, but they fell of a few weeks ago

that is a root forming which is good....repot it...it looks like it got sunburned, did you have it in direct sunlight? it looks like you watered it and put it in the sun to me...indirect light is what phals need...i think the main stem is healthy, so repot it and keep it in shade only....water when it is almost dry, dont let it go competely dry before watering it. when you water, water thoroughly, letting the water flow thru the media untill it is soaked thru...the ugly leaves can stay till they turn completely brown, they are still capable of sustaining the plant till new ones form....gl

Okay, I did not see these pics when I posted. I think the roots look okay. Yes, that is a new root. And it may very well be sun damage, and if that is true, you may be able to salvage this plant. Sorry for saying it is dying, but from the looks of that first picture, it really looked like the crown was affected and black.
This is why it is so great to have other opinions.

what do you recomend repotting it in, and should i repot in a samller, bigger, or the same size pot

Not in soil. You need to get orchid mix, which is not soil at all, but a bark mix. Soak the mix for 24 hours in water, then rinse, and it will be ready to use. Be sure to clean your orchid pot good in hot soapy water, if you plan to reuse it. You need an orchid pot that has plenty of holes...if you get a new one. In the second picture (in the set of three pics), it looked like a black root or stem. You should cut that one using a sterile knife or scissors.
Before replanting, swish the roots in listerine (plain not minty), then repot.
Phals like indirect light, but keep it out of light for a week or so until it can adjust to the repotting. Good luck to you.

things are looking better and worse.
the crown is gtting worse. but there is some awesome new root growht and something that looks like a keiki or a spike
|
