Default Beginner needs some help with ID

Hi, I am new to orchidmania and I have discovered that browsing this Forum is just as addictive as collecting and growing orchids. It's great!

My wife has had this orchid for many years and only recently has it bloomed. It was given to us by a botanist friend and we think it was collected in Florida more years ago than I like to remember. Can anyone help with identification?

Also, recently the leaves have withered on the tips and I am afraid I have done something evil. Any comments or advice would be appreciated.

Maybe you're giving it to much fertilizer or sun. Other than that it looks ok.

Thank you... I searched an orchid encyclopedia and found a lot of information about the distribution and ecology of the species. And a LOT of taxonomic changes.

Again thanks. I will monitor the fertilizer input more closely. I think the light has been pretty moderate.

It's also know as the "clamshell" orchid (as you will see from the link that Dorothy provided) I have one as well...very nice plant you have and you've done a great job growing it!
