It looks like it might be one of the Oncidium group hybrids. While the visible roots look OK, the mix looks really wet. I strongly suspect that it needs repotting (small bark is good). The older pseudobulbs probably have dead roots, hence the shriveling... they can't take up water, so they serve as "reserves" for the rest of the plant until it grows some new ones. Can't tell from the photo, but if there are bugs, then clean them off with soapy water (which will also help kill them) The black spots I would not worry about. Just wear and tear. So in short, pot into fresh media (the existing pot should be fine), cleaning up dead roots and wet, broken-down media. Then put it in a somewhat shady spot, keep it moist but not soggy-wet (let water run though the pot, which pulls air into the roots zone) and let it grow. It will probably start a new growth if it hasn't already.
