A. It is a 'Maudia' type hybrid (or species). If from a mass producer, it is 99.999% certain that it is a hybrid. Even with a flower photo, it would be impossible to identify it further (unless it is a species).
B. Since it has few roots, plant in a material that is easy to keep moist, but not soaking wet. Place it in low light & high humidity, and make sure that it NEVER dries out completely.
To maintain high humidity, you might need to place it inside a clear plastic bag.
If you have any clear plastic pots of a suitable size (with Paphs, underpot rather than overpot), use it so you can see when you have new root growth.
C. If the plant tries to flower before it grows new roots, remove the bud. Plants in poor health often try to reproduce, thus hastening their demise.
