Looks like a peloric Miltassia (Bratonia) Charles M. Fitch 'Izumi'. I read somewhere that it was a mutation while cloning and now has its own name 'Three Lips'

Sweet! That's definitely it, someone had suggested Charles M. Fitch but it didn't look like it. But the 'three lips' version is the one!

I second the peloric ID, if the label/petals are identical (which means peloric in the orchid case).
