I'll stick with my P. cornu-cervi suggestion. P. amboinensis has an oval lip, whereas P. cornu-cervi has a winged lip as seen in his photo. Plus P. c-c is native to Thailand where he took that photo (and it looks like the P. c-c I have growing in my backyard)

Not sure who sells it stateside, but there are several growers here in Chiang Mai that ship to the states.

P. amboinensis has a broader flower shape too, and doesn't have the flattened flower spikes. I guess if it is a hybrid that could be a candidate for the other parent though!

Another couple of things to throw in: I'm not sure if this is true of ALL cornu-cervis (I'm guessing possibly not flava ones, which this isn't anyway), but it's typical for the mature leaves to have a very fine dark line down the centre. There might also be a fine sprinkling of dark dots, like a bit of black pepper. These features are too subtle to show in the picture.

This is not amboinensis, nor is it likely a hybrid with ambo either. This has very little features that are not cornu-cervi.

Jzorn:
Have you tried the website Asiatic Green? They list a ton of species orchid and list the one you mentioned last. I have not bought from them, but you may give them a try. They list Phal. amboinensis for $25.00 currently. They are an asian distributor, but list plants in US Dollars, so they obviously sell here. Now whether they handle the CITES paperwork or leave that to you, I am not sure, but it is a fun site to visit.

Definitely cornu-cervi. There are at least 3 color forms of c-c, and some variation within each form. I think Big Leaf has the red form on their website.
