some sort of den-phal?
Default some sort of den-phal?

I purchased this several months ago and was curious to know more specifically what it was.

I would sure appreciate any attempts at IDing this as well as any input on care for it (I might be doing something wrong). This one also has to new shoots or canes coming up but no other spikes yet. It has also had two of the leaves near the middle turn yellow and fall off plus there is one now at the top that seems to be developing a hole...?

FYI, I am keeping it near an east window and water it about once a week. I basically water it until i is running out the bottom and don't water again until it is fairly dry. It is in an orchid mix with rock gravel at the bottom for drainage.

I have one that looks like your that is labeled Den. Emma Queen -

Its all white on the back of the flower too. The petals usually aren't as curled back as they are in this picture but I took late in the flower cycle.

You must be doing something right with new growths coming in. Hard cane dens will lose their leaves as the canes age, eventually going completely bare.

The hole in your leaf may be caused by slugs or snails or, more likely, from water sitting on the leaf too long. Mine grow outside year round and they get holes in them from time to time. Best not to allow water to stand of the leaves over night.

Mine get about 3 hours of morning direct light and I'm watering once a week as well.
