Noticed this the other day, any guesses as to what this is or how it happens? Is this common amoung hybrids?

Have you been applying any rooting hormones? Overdoing them can lead to flower deformation.

No, rooting hormones. Have done nonthing different with this phal. than with any other phal. we have. Same water, same fertilizer, same light and temp. Nonthing different
