Question Dying Orchid?

I purchased a Phalaenopsis orchid on February 20, 2015. Now it's April. I've been taking care of my orchid pretty well (at least I think I have), but just recently, my orchid has started losing leaves. It started out with about six or seven leaves, but one day, one of its leaves got scratched. In about a week or so, the leaf yellowed and died. Some of the other leaves also began yellowing too. The new leaves fell off my orchid. Now I only have two leaves left. One of the last leaves of my orchid seems healthy to me (no bumps, yellowing, stuff like that), but the other one has begun yellowing. The orchid's blooms are still pretty healthy. Do you think my orchid will survive and grow new leaves? What can I do to help it? (BTW: Some of the roots were dried up when I first bought my orchid).

You need to post some pictures of the leaves you think are healthy. What I'm seeing doesn't look too hopeful.

To me (in the first picture) the base of the plant looks rotted. It's better if you don't water so close to the core of the plant and just get the roots wet for phals. This plant may be beyond help at this point.

Are you by any chance using the "ice cube method" of watering?

How was the plant protected when you brought it home from the store? I'm willing to bet that Feb. in Wichita was pretty cold. This could be a delayed reaction to cold stress, but I would have expected the buds to blast as well (unless they wrapped the spike but not the plant).

The buds did blast, but I didn't water my orchid with ice cubes.

The orchid was in some kind of plastic wrap. The breathing hole was at the top.
