The top photo shows the beginnings of a "keiki" (KAY-Kee - Hawaiian for "baby"), while the lower shot might just be a new branch off of the inflorescence.

Some phals are more genetically predisposed to do so, and form offshoots naturally, but in the larger hybrids, it is sometimes an indicator of something lacking in their culture.

If your Phal looks healthy and blooms I don't think the keiki is due to any cultural problem. Sometimes a dying plant will get keikis in an attempt to survive. Some Dens, and perhaps some other types, can get keikis instead of blooms if they got fertilizer/too much water during the winter rest period.

The plant is looking really healthy with two full grown spikes! Really green healthy leaves! I hope it won't wither!

The keikis are now growing. No roots coming out yet. How do I care for them? Do I cut them off now? I wait for the roots? Should I fertilize them more intensively ?

I believe that for now you just care for the mother plant as you normally would. As for when to remove the keiki you should follow the 3" root rule, wait until you have either 1 root that is 3" or 3 roots that are 1" or any variation in between. I've read of fertilizers out there that encourage keiki root growth however I have never used any so I'm not sure if they are worth the extra cost.

Right now I have a phal that has been working on a basal keiki for over 4 months now, it is still so small although it is easily twice as big as it was when I first discovered it. Everyone on here isn't kidding when they say that phals are slow growers.
