Phal plantation yellow bells

everyone,

I bought this orchid in the spring; it was shipped bareroot. I have it in a 4" pot in seedling bark. I water every 3 or 4 days; it gets good eastern exposure part of the day. It hasn't gotten any new growths or a spike, what is up with it???? Thanks for any help. I have included several pics of it; the last pic is where I have it located; as you can see it gets a fair amount of bright light. Also the lower leaf on the right is fading out; I don't know if this is natural due to it being older or what, but the other leaf on the left did this as well so after it turned yellow I removed it. It just seems to be stuck in limbo, I don't know what's up with this thing, maybe if I stick it next to the garbage can it will get the hint bahahahahahaha
