Exclamation "healthy looking phal - no roots" HELP

Had a healthy phal - new growth, several years old - but looked at it tonight and since one leaf looked a little puny, pulled it out of the pot to look at the roots. I wish I'd looked earlier. Had no idea that it was in this bad shape. I'd repotted it about three months ago in bark. I was keeping it in an indoor greenhouse with around 90% humidity, good air circulation, and temps ranging from 70 to 80. It was also in a clear plastic pot with excellent drainage. Any ideas of what to do to possibly save the plant? Is it possible? Any idea of what went wrong? Is it possible to have too much humidity?

I'm no expert, but some people have mentioned to pot it into the smallest pot possible in fresh medium.

I am also no expert and just a junior member for this board. I have three phals and one of them went through terrible root rot. It was left with only one good root. I potted it in fine orchid bard with 1/3 part perilite for drainage. It is now slowly recovering. It grew 4 new roots after a few months and now is growing another. It also won't stop pushing out leaves (although still small in size).
Anyway, I would try putting it into a pot with some wet spag moss. Since it has no roots, just put the long leg- like stalk that used to have roots into the moss. And cover any areas that you would like to have roots with the moss. I would make sure to mist the moss regularly. Maybe that will help it grow new roots? I'v read that in a book... Hope it helps. Can you post some pics of how it looks now?

You can save this plant by repotting it into a small pot (2 or 3 inch, just enough to fit the stem, or whatever is left of the roots) with sphagnum moss. Moss should be soaked overnight - then squeeze out all the water. This way moss is moist, but not wet. Mist the moss daily. Your plant may not have root now - but it will grow new roots overtime.

Thanks for all the info. I finally gave the orchid to the Huffmans at Jolah Orchids. Jim says that he likes projects like mine so he now has it.
