I agree with Rob....gazillions of these are produced and most never have a name. I'd be interested where you live, Herf, 'cause the Sam's Club here in West Bloomfield, MI have the exact same plants... no wonder, since they probably buy for all of their stores nationwide and distribute to all.... BTW, the grower on the ones at Sam's Club is the same as at my local Costco.....Costa Farms with home offices located in SE Florida and GH's elsewhere.

A quick web search for Phal. Silibama turns up that the plants name is P. Silibama. I can't help with the number after it, it probably is an order number or number given to identify the plant in a data base somewhere.
P. Silibama still looks like it must have some species Phal. amabilis in it.......that would also explain the name......

There is a picture of it at this address:

LITTlEFROG: You are probably right about the clonal name for Baldan's Kaleidoscope, they have been mericloned to death. LOL

I'm 99% sure about the harlequin. P. Ever Spring Fairy 'Shan Pin'. It was printed on the pot and I really did spend hours verifing it.....

Herf,
It just struck me (I'm a bit dense sometimes) but your P. Silibama is amabilis spelled backwards.......more reason to believe that P. Silibama is a hybrid of the species P. amabilis.

Costa Farms is also the supplier of Home Depot way up here in the willy-wags of Maine. They also supply all the tropicals for "our" H/D.
