It looks beautiful! I would guess it has Den. phalaenopsis related species and antelope/spatulata species in its ancestry. It will like warm temperatures, bright light and regular water all year. There is a good chance it will become much taller, and branch profusely from the base.
With plant like yours I would never unpot and remove old medium. The old roots aren't doing much, anyway, and Dens like this form lots of new roots at the growing point. Just find a pot about 4" / 10cm larger in diameter. Drop your plant, in that pot, into the new pot. Backfill with medium or large bark chunks, depending on how you water.
Most of the time I wouldn't repot any Dendrobium unless it's just forming new root nubs. In this situation, though, dropping into a new pot will not disturb existing roots, so you could do it now.
