This is a clear case of a tag that just got made too hastily for basic accuracy checking. That should say Paph. Hilo Citron. That is a legitimate name. There is no Den. Hilo Citron, and such a cross would be impossible anyway.
The tag indicates a cross between two Paphs. That cross has not been given its own name yet, so the tag gives full identification of both parents.
Registered cross names are selected by the person who pays to register them, with the permission of the person who originated the cross if it is someone else wants to register it, and with the approval of RHS. That name then applies to all plants that result from crossing those 2 parent species or hybrids.
'Cultivar Names' in single quotes are given to a single plant, and can be given by anyone at any time as long as there hasn't been a previous cultivar name given. The cultivar name then applies to all divisions or clones of that one plant.
