When Paphs show the pattern of growth you have described it is natural to assume they are getting a second spike, but that isn't the case. It would have been a 2 flower spike, but got stages of growth out of order. Instead of the base of the stem below the first flower elongating it stayed very short and the first flower emerged almost among the leaves. They are often deformed when that happens. Then the normally short segment of stem between the 2 flowers started elongating more than usual, appearing like a second spike. It happens sometimes especially when the plant gets a shock of some kind in early stages of spike development. It will probably flower normally when the next growth matures..
