...just looked at your picture again... it seems as if that plant doesn't like CHC. There are two leaves that shouldn't be dead, and another leaf that is browning at the ends. You may want to consider repotting now or after the plant blooms into some other medium. Perhaps a nice sphagnum moss with a fine bark media on top would work nice...

The plant wouldn't loose or stress the spike if you repot at this time, they are pretty resiliant... If you repot when there is a flower or buds that are visible, then you may loose that bud or flower, but usually not.

CHC!....coconut husk chunks? Is that a medium that does not appeal to Phrags in general? Seems like the water holding ability of it would be good for them.

I use CHC on ALL my Paphs and Phrags. As long as it is leached properly there is nothing wrong with it. In fact any medium would retain salts without leaching...even sphagnum and bark.
