OW, I just posted on this, couple days ago. You might check that post. Mine took a year off, it needed more light. So, your light is good. I read that they like to be abused a bit to induce flowering. I withheld water for about a month last winter. They love fertizer and fresh medium ( I read) and mine exploded after the drought and then good soaking and fertilizer. I now have about 4 flower heads and more coming. They will send up something that looks a little like a leaf but if you look against the light you will see the flower head inside. Hope this makes sense and helps.

Welcome to the Orchid Board!

Is it possible spider mites are killing off each new growth? The leaves get a silvery sheen, then dry up and fall off.

Mine is in bark. It is now sending up lots of shoots and a few keikis and lots of adventegous roots. Honestly, it has thrived in benign neglect. It has been in nested with snails, set out in full sun all summer and suffered drought. Maybe you are too nice to it? 😉
