It's on the same plant. It just started opening this morning, so it might not be fully open yet.

Are the lower two sepals still joined together under the lip as they are in the picture?
It looks like there is a slight mutation if it's not opened fully and the lip filled out. I wouldn't worry, it happens in orchid flowers sometimes.
