oh, well...
I don't know why your sheaths would die out like that. Too much nitrogen comes to mind as a possible cause, but I don't know what your nutrient regime is. Maybe a little less N and a little extra P leading up to sheath production.

I actually give little to no fertilizer. lol
Maybe I need to up the feeding.
How do you feed yours?
