It is difficult to know without a few questions, first. What is your lighting? My catts get strong south-window light in winter and go outdoors in the summer. Do you flush your pots to eliminate fertilizer residue from concentrating? Does your water provide the important micronutrients missing from your fertilizer that plants all need? How are your roots? Do you repot when the bark gets decomposed? (I dislike repotting so I mount or use red lava rock for most of my orchids). What are your temps? My home is ~64'F so I don't fertilize from Nov until Feb. except to make certain the catts have a source of calcium (I use egg or oyster shell). During the summer, I will now use an MSU fertilizer but, in the past, I had to use suppliments to keep my catts happy. I hope this helps. It is how I grow most of my catts. Good luck!
