Your photos are badly out of focus. You probably have a camera that cannot focus from as close up as you attempted.
From what I can see (and conjecture the rest) from what you have said. I think that you don't have very many good roots. I can see 2 new roots starting up ... maybe an inch or two. But for a s bulb plant, there should be more roots showing.
On the right side of the photo, at the base of the plant, there appear to be some roots (photo's blurry so it's hard to tell). But if they are roots, they are very short & seem to have brown tips ... tips are dead. Back of the plant seems to be potted deep. (another hint that you don't have enough roots to hold the plant straight up)
If I'm guessing right about the lack of viable roots, then, it's not surprising you have a dead new eye. Check your roots first. It's hard to tell beyond that since your photo doesn't show enough.
